Purpose Statement
1. To promote and ensure safe work practice and conditions at the worksite
2. To ensure the requirements of WCB OH&S Regulations are met
3. To ensure the requirements of School District No. 50 (Haida Gwaii/Queen Charlotte) District Health and Safety Program are met
Composition
1. At least four members representing workers and management
a) at least 50% shall be worker representatives
b) quorum shall be a minimum of 50% of representatives, with a minimum of one management and one worker representative
2. Co-Chairs
a) one selected by the worker representatives
b) one selected by the employer representatives
3. Recorder
a) to be selected by mutual agreement at each meeting
Procedures
1. September Meeting
a) review School District No. 50 (Haida Gwaii/Queen Charlotte) School Site Committee
b) confirm Site Committee membership and co-chairpersons
c) confirm a designated First Aid Attendant (FA) and an alternate
d) set meeting dates for the year
e) set inspection date for the year with at least two during peak activity periods
f) review concept of Maintenance vs. Safety
g) review WCB OH&S Regulations book
i) layout and use of regulations
ii) site committees and responsibilities
h) ensure the following Health & Safety materials are placed in a clearly marked publicized location readily accessible to employees and management:
i) WCB OH&S Regulations book
ii) copy of current and prior year inspection reports and minutes
iii) other relevant materials and publication including: Shop Safety, Science Lab Safety, Heads UP!, WHMIS, MSDS
i) ensure Health and Safety is designated as a permanent agenda item at monthly staff meeting for reporting and information purposes
j) complete all requirements of the Regular Monthly Meetings as stated under section 2
2. Regular Monthly Meetings
a) review previous Site Committee Minutes
b) review previous safety report
c) complete current safety report School Safety Inspection Form
d) review and carry forward prior to items not remedied
e) send minutes and report to District Health and Safety Committee
f) inspection and monthly meetings to be held the second week of each month
g) report to the District Health and Safety Committee by the third week of each month
3. Accident Investigation
a) include list of all current accidents and fill out investigation forms for each accident
* if a Violent Incident has occurred please complete the following form in addition to the WCB forms below – Violent Incident Report
b) report accidents with appropriate WCB forms as per WCB Act
i) employee to fill out Form 6a, & Form 6 send to WCB and a copy to the District Office
ii) the District Office will fill out the Form 7 and forward to the WCB
4. Inspections
a) review previous safety report prior to the inspection
b) allocate inspection responsibilities
i) no fewer than two shall inspect areas of the site
ii) rotate areas of responsibility
c) visit the site’s work areas
i) check for items listed on previous safety reports
ii) record all safety concerns (get Safety Inspection Form here)
5. Duties & Functions
a) Chairperson
i) chair meeting
ii) ensure site committee is taking all reasonable precautions to comply with requirements of WCB and District Regulations
b) Recorder
i) record minutes
ii) ensure minutes and inspection reports are completed and submitted to site administrator
c) Site Administrator (normally Principal or Vice Principal)
i) primary responsibility to ensure Site Committee is formed and functioning
ii) Authority:
Principals/supervisors are responsible for ensuring compliance with WCB regulations and District Health and Safety Program regulations. They will work closely with School Safety Committees to eliminate health and safety concerns and to promote health and safety inservice activities
iii) designate Health and Safety as a permanent agenda item at monthly staff meetings for reporting and informational purposes
iv) ensure minutes and inspection reports are completed and submitted to the District H & S Committee
v) ensure minutes and inspection reports are maintained in a secure location
vi) ensure health and safety concerns raised are dealt with in a timely manner
vii) ensure that at least two inspections per year are conducted during peak site activity hours
6. Records
a) all inspection records, notes, reports and minutes for the current and previous year shall be maintained in a secure location
b) the District Health and Safety Committee shall retain site committee reports and minutes for a period of three years
